Dyslexia Services Foundation (DSF) Grant Application
WE ARE NOT CURRENTLY ACCEPTING APPLICATIONS
Grant Requirements and Stipulations
DSF only funds individuals or organizations that (1) provide treatment services to dyslexic children who are otherwise unable to obtain such services and/or (2) promote public awareness of dyslexia.
DSF requires that grant applicants submit this application form including a grant proposal. All proposals are reviewed for merit by the DSF Board of Directors to determine if the application serves the mission and exempt purposes of DSF.
If a grant proposal is accepted, the recipient and DSF will execute a written agreement that establishes the terms and conditions of the grant and specifies the responsibilities of DSF and the grantee. The agreement will obligate the grantee to use the grant funds for the purposes for which the grant was made. The agreement will also acknowledge the authority of DSF to withhold and/or recover grant funds if such funds are misused.
Grantees are obligated to provide periodic reports as well as a final written report concerning the use of grant funds. Grantees will be required to keep records pertaining to the use of grant funds and DSF will have the authority to conduct an audit of grantee records.
